I’ve spoken to about 6 people when I called the passport agency and everyone had different responses. On the DS64 form there’s a window where you write in what happened to your passport. Hi Ira, I just submitted the DS-64 form (lost or stolen passport) along with the DS-11 and my child’s original birth certificate and of course all the fees again for expediting and passport. I have also contacted my congressman and they contacted me back and were working on my issue, I’m not sure if that helped but it’s worth a try.
